The Quality System Paradox

Somewhere along the way, complexity became confused with quality.

In regulated environments, additional procedures, approval steps, and documentation are often seen as evidence of stronger control. The assumption is understandable. However, complexity alone does not guarantee better quality outcomes.

Too often, the result is a system that appears robust on paper but is difficult for people to navigate and apply in practice. So they invent their own workarounds, putting the company in automatic noncompliance with its own system.

Quality systems are essential. They provide the structure, governance, and oversight needed to ensure activities are performed consistently and in compliance with regulatory expectations. However, when the focus shifts from enabling good practice to simply meeting requirements, quality can become an exercise in administration rather than a driver of effective operations.

Over time, many organisations invest significant effort in building increasingly sophisticated systems, only to find that those systems have become a barrier to the work they were intended to support.

A gold-plated quality system attempts to document every possible scenario, introduce controls for risks that may never materialise, and establish governance structures for challenges that do not yet exist. And isn’t in line with the risk-based approaches being championed by the global regulators.

For smaller or growing organisations, the consequences can be significant:

  • Procedures become harder to maintain.
  • Approval processes become slower.
  • Documentation becomes increasingly disconnected from operational reality.
  • People spend more time navigating the system than using it to support their work.

Complexity alone is not a measure of maturity.

More documents do not automatically create better quality. More approvals do not automatically create stronger oversight. More controls do not automatically reduce risk.

The critical question is not:

“How much can we include?”

It is:

“What level of control is needed to manage risk, meet regulatory expectations, and support effective operations?”

Fit-for-Purpose Quality Systems

An effective quality system reflects an organisation’s activities, risks, regulatory obligations, and operating environment. It provides the right level of control for today’s needs while remaining flexible enough to evolve as those needs change.

Scalability is a critical part of this. A quality system should not only support the organisation as it exists today but also provide a foundation for future growth.

For example, a company may not be operating under GxP requirements today but may aspire to do so in the future. Rather than implementing a fully GxP-compliant system prematurely, it is often more effective to establish a framework that can evolve when the need arises. This means thinking ahead without over-engineering and building flexibility into the system rather than rigidity.

A useful way to assess whether a quality system is fit for purpose is to ask three simple questions:

  1. Is it compliant? Does it meet applicable regulatory and business requirements at this time?
  2. Is it usable? Can people understand it and apply it consistently in day-to-day operations?
  3. Is it scalable? Can it evolve alongside the organisation as activities, risks, and regulatory expectations change?

One size does not fit all. A global organisation will naturally require different controls from a growing business establishing its first regulated processes. The objective is not to build the most comprehensive quality system from day one, but to build one that is appropriate for the organisation today and capable of evolving as it grows.

Quality Systems Are Built for People

The most effective quality systems recognise a simple reality: organisations are made up of people.

The purpose of quality is not to eliminate human judgement. It helps you make better decisions by providing the right level of structure, guidance, and control.

A well-designed system creates consistency where it matters, provides clarity where decisions are complex, and establishes appropriate oversight where risk demands it. This is why practicality matters.

A quality system must work in the environment where it is used. If a procedure bears little resemblance to how work is actually performed, it will not improve quality.

If a process is overly complicated, it will not create sustainable compliance.

If users do not understand or trust the system, it will not achieve its intended purpose.

Quality by design is not about creating a perfect environment where nothing ever goes wrong. It is about creating an environment where risks are understood, managed, and continuously improved.

The strongest quality systems work with people, not against them.

Quality Is Everyone’s Responsibility

Quality is often viewed as the responsibility of Quality Assurance (QA), but quality cannot be owned by QA alone. QA plays a critical role in providing oversight, guidance, challenge, and assurance. However, the responsibility for delivering quality outcomes sits across the entire organisation.

Every function contributes to quality through the decisions it makes and the way work is performed. Scientists, operators, engineers, managers, and support teams all influence quality by understanding risks, following appropriate processes, and making informed decisions within their areas of responsibility.

The role of QA is not to create quality on behalf of the organisation. It helps teams understand risk, establish appropriate controls, provide independent oversight, and support a culture of accountability.

When quality is embedded into everyday operations, compliance becomes a natural outcome of how work is performed rather than an additional activity managed separately.

Quality is not separate from operations.

Quality is how operations are performed.

How Do We Measure Quality?

A quality system should not be judged by the number of procedures, policies, or records it contains.

Documentation is important, but documentation alone does not create quality.

The real test is whether the system consistently helps people perform their work effectively, safely, and compliantly.

Some useful indicators include:

  • Consistent process execution
  • Effective management of deviations and CAPAs
  • Reduction in recurring issues
  • Strong inspection and audit performance
  • Meaningful employee engagement with quality processes
  • Evidence of continuous improvement

Ultimately, the question is simple:

Does the system help people make better decisions and manage risk effectively?

A system that appears impressive during an inspection but struggles during day-to-day operations is not truly effective.

Final Thoughts: Build Quality That Works

Building an effective quality system is not about creating the perfect system from the beginning.

It is about creating a system that works for the organisation today while providing a foundation for tomorrow.

Whether building a quality system from scratch or improving an existing one, the objective should be the same: design quality that reflects operational reality, not theoretical perfection.

Just because something worked in the past does not mean it remains the right solution today. Processes should be continually challenged, refined, and improved as organisations evolve. Equally, compliance alone should never be mistaken for effectiveness.

The best quality systems balance:

  • Control with practicality
  • Structure with flexibility
  • Regulatory expectations with operational reality

They provide the right level of oversight without creating unnecessary complexity.

Because in today’s regulatory landscape, the best quality system is not always the most sophisticated.

It is the one that people understand, use, and trust.

It is the one that works.
